Printable Order Form(s)
The Order Forms are HTML files, and require that
the user right click on the face of the form and select Print to print
it. To open the DVD Order Form file, click here DVD Order Form, to open the
Photo Order Form file, click here Photo Order Form.
The user may type entries into the text boxes on the Order Form(s), then
print out the form(s), or may print out the form(s) and write in the
entries, whichever is more accomodating. Please note that an Order
Form is not necessarily required as long as you iterate what items you
would like to order and the proper pricing/payment for those items, but an
Order Form may simplify the process, as it is already setup with item
listing, proper pricing, and other information.
